Information & Assistance
- Information & Assistance (I&A) offers tailored support to older adults (60+) and individuals with disabilities, as well as their families, friends, caregivers, advocates, and other representatives.
- The service provides extensive information on community resources and public programs.
How it Works: Anyone in the community can contact the Aging and Disability Resource Center (ADRC) with inquiries about resources to promote the independence of older adults and individuals with disabilities. ADRC staff will supply specific information and/or a comprehensive resource guide of available services.
LTSS Options Counseling
- Options Counseling is a collaborative process designed to guide and empower individuals and their families in making informed decisions about benefits, services, and supports.
- This assistance involves identifying and prioritizing goals, as well as coordinating access to both public and private long-term services and resources.
- Available resources and options may include in-home care, community services, care management services, residential care, and skilled nursing care.
How it Works: Long-Term Services and Supports (LTSS) Options Counseling involves working with an ADRC staff member to explore services tailored to your specific needs. This may include transportation, home care, health, medical, social services, and more.
Short Term Coordination
- Short-term services coordination ensures quick access to necessary services and supports for individuals at risk of institutionalization or hospitalization.
- Priority is given to those who urgently need services and programs to maintain their health and safety.
How it Works: Short-term coordination involves working with an ADRC staff member to create a plan that connects you to eligible, fee-for-service, and/or free services and resources available in the community.
Transition Assistance
- Transition Assistance aids individuals currently in hospitals, nursing facilities, or other institutions who wish to transition back to their homes or communities.
- This support encompasses information and access to a comprehensive range of resources and services to facilitate the transition process and address in-home care needs, including assistance with paperwork, planning, and advocacy.
How it Works: Transition Assistance offers guidance from an ADRC staff member to help individuals move from facility care to their homes or a less restrictive living environment.
